To play, you must own Clinic Deluxe Edition. This box contains the Big Box organizer (5 empty trays) + the 911th expansion. Other Clinic expansions are not included: the trays are designed to accommodate them, but they must be purchased separately.

Clinic is one of the best management games to play solo. The 911th amplifies the solo experience: the double building puzzle requires fine planning, and without the interference of other players, you can truly optimize every move. Still requires the base game to function.

What changes with the second building
Not the rules. The experience after adding the module.
Doubled setup
Pull out the 911th tray. Place the second board to the right of the main one. Shuffle the new doctors, new nurses, new treatment rooms. The table expands. Someone realizes they need more space. Someone looks around and says: "Alright, let's start."
First time thinking in three colors
In the base game, you matched the doctor's color to the patient's color. Now a red patient arrives in a yellow treatment room, and you realize you also need a doctor whose specialization matches the room. Three dimensions instead of two. Your brain clicks audibly.
The nurse who saves everything
The 911th introduces a new type of nurse: she covers color mismatches. When the patient is red, the room is yellow, and the doctor is blue, she is the difference between successful treatment and a logistical disaster. You realize she is the most valuable asset in your hospital. You also realize you've only hired one.
Flow between the two buildings
Mid-game, you manage two hospitals simultaneously. Patients arrive from both entrances, move between facilities, and some treatments are only done in the second building. Three-dimensional matching is everywhere. Whoever plans the flow between the two boards best wins. Those who treat the two buildings as separate lose.
Counting and silence
Points are counted. Rooms built, patients treated, objectives completed, efficiency bonuses. The winner almost always has a functioning second building. The loser left it empty, or wrong, or built it too late. Everything is put back into the Big Box trays. It's surprising how fast it is.
What the module adds
What changes compared to the base game
The 911th is not just a simple card addition. It redefines matching rules and introduces a second parallel flow. Four central changes.
In the base game, you matched doctor color — patient color. Now you also add the treatment room color. Three variables to synchronize; each treatment becomes a mini-puzzle. Building choices now weigh more than twice as much.
The second building has its own separate board. It is built with rules similar to the first but with distinct flows, its own entrances, and calibrated capacity. It functions as a satellite to the first hospital or as a specialized hospital — depending on how you build it.
A new staff member: the nurse who fills color mismatches. She costs money, must be managed, but transforms impossible treatments into possible ones. She quickly becomes the most contended resource of the round.
Traditional operating rooms are joined by new treatment rooms, each with a color. The color is binding: certain patients can only be treated in certain rooms, and only by compatible doctors. Building the wrong room is an expensive mistake.
